We may be able to help you access additional resources to help your family thrive! Through Healthy Opportunities, we can help Medicaid Managed Care recipients to get food for their families, stable housing, reliable transportation, and support for personal safety.

Who Qualifies?

Medicaid Managed Care members with a qualifying health condition and at least one social need, living in Western North Carolina. If you have Medicaid Managed Care, your insurance provider is one of the following companies:

• AmeriHealth Caritas

• Healthy Blue (Blue Cross Blue Shield)

• United Healthcare

• WellCare of North Carolina

If you are on NC Medicaid Direct you do not qualify at this time.

• Healthy Food Boxes

• Healthy Frozen Meals

• Moving Expenses

• Rental Deposits

• Healthy Home Goods

• Accessibility Modifications

• HCM can help you access additional HOP Services
